Event Description
We believe that the Cancer Rehabilitation and Personalised Care Community of Practice offers a unique opportunity for passionate people to explore and reflect onour own practice in light of the practice of colleagues. We hope the community willhelp us to learn with and from others and help us all to consider approaches that may work, in improving our patients rehabilitation and personalised care experience.

Why a Community of Practice (CoP)?
- We will bring together those with a passion for improving rehabilitation and personalised care for our cancer patients in South East London
- We will create a space to discuss challenges and opportunities in delivering rehabilitation, and work together to find innovative solutions.
- Our CoP will support you to develop cross boundary relationships
- and tap into the collective experience and intelligence of the community, turning 'promising practice' into 'shared best practice'
